Request official LSP membership documents such as your Membership Certificate, Membership ID, and Official Receipt based on the information currently available in our database and the details you previously submitted.

​This service is available to both active and expired members, provided that their membership records are complete and the required specimen files (such as photo and signature) were previously submitted and are available in our records.

Eligibility and Important Reminders​

• Priority processing is given to active members.

• Documents will only be processed if the submitted information matches our records and all requirements are complete and in order.

• Expired members without previously submitted specimen files (photo and signature) are not eligible for the full document package.

• Specimen files cannot be uploaded retroactively for expired memberships. Members without complete records may need to apply for a new membership.

• Processing time is approximately 3–5 working days, or earlier if the documents are readily available.

• Approved documents will be sent only to your registered email address.

• For privacy and security reasons, we cannot send documents to other email addresses.

• If you do not receive a response, the email address provided may be incorrect. Please contact LSP at for assistance.

• This request is for document issuance and survey purposes only and does not extend, renew, or reactivate membership.
